How assessment pricing works

Evaluations are the standard initial visit. An ADHD, anxiety, depression, bipolar, PTSD, or postpartum evaluation is a 45–60 minute diagnostic visit priced at $179. A second opinion is the same visit with a written summary. Follow-up medication management visits are $129.

ESA letters and FMLA paperwork have their own flat fee. An ESA letter evaluation is $249, and FMLA or short-term disability paperwork is $249 per form packet. Accommodation letters are $179. These are self-pay and are not billed to insurance.

Testing is billed by the lab. Pharmacogenomic testing is ordered through a laboratory that bills you or your insurance directly; the results review with Angela is a $179 visit.

Payment is due at the time of service. Major credit cards and HSA/FSA cards are accepted. Commercial insurance may cover diagnostic evaluations; verify outpatient mental health and telehealth benefits with your plan before booking. Medicare, Medicaid, and AHCCCS (Arizona Medicaid) are not accepted; patients with those plans are still welcome to book as self-pay. See the insurance and pricing page for details.

New patients versus established patients

Diagnostic evaluations, the second opinion, and the ESA letter evaluation are open to new patients. Accommodation letters, FMLA and disability paperwork, and pharmacogenomic testing are for established patients only. The reason is credibility: a disability office or employer expects documentation from a provider who has evaluated and is treating the condition, and a letter written at a first meeting is the kind that gets questioned. If you need one of these documents and are not yet a patient, the path is an initial evaluation first.

What is not offered

Honesty about limits is part of a credible assessment practice. Sigmon Psychiatric does not provide full neuropsychological or IQ testing, autism diagnostic evaluations (ADOS-2), FAA or DOT evaluations, immigration or hardship evaluations, VA disability nexus letters, custody or court-ordered evaluations, or firearms-eligibility opinions. These require a psychologist, an in-person protocol, or a forensic specialist, and Angela will say so rather than produce a document that would not hold up.

Assessments and letters are outpatient services, not emergency care. If you or someone with you is in immediate danger, call 911. For a mental health crisis, call or text 988 or Arizona's statewide crisis line at 1-844-534-4673, both available 24 hours a day.

Frequently asked questions

How much does a psychiatric assessment cost at Sigmon Psychiatric?

Condition evaluations (ADHD, anxiety, depression, bipolar disorder, PTSD, postpartum, second opinion) are $179 as a standard initial evaluation. An emotional support animal letter is $249, an accommodation letter is $179, FMLA or short-term disability paperwork is $249 per form packet, and a pharmacogenomic results visit is $179 plus the lab's own fee. Prices are self-pay; HSA and FSA cards are accepted.

Can all of these assessments be done by telehealth?

Yes. Every assessment listed is completed by secure video with Angela Marton, PMHNP-BC. You must be physically located in Arizona during the visit. Services that require in-person testing, such as full neuropsychological testing, autism diagnostic evaluations, FAA pilot evaluations, immigration evaluations, and court-ordered or forensic evaluations, are not offered.

Do I need to be an existing patient?

Diagnostic evaluations, the second opinion, and the ESA letter evaluation are open to new patients and can be booked online at sigmonpsych.com/book. Accommodation letters, FMLA and disability paperwork, and pharmacogenomic testing are for established patients, because the documentation has to rest on an evaluation and ongoing treatment.

Does insurance cover assessments and letters?

Diagnostic evaluations may be billable to commercial plans; verify outpatient mental health and telehealth benefits with your insurer first. Letters and paperwork (ESA, accommodation, FMLA) are self-pay and are not billed to insurance. Medicare, Medicaid, and AHCCCS (Arizona Medicaid) are not accepted; patients with those plans are still welcome to book as self-pay.
