Work and School Accommodation Letters in Arizona — ADA and Section 504 Documentation

An accommodation letter documents a diagnosed mental health condition, the ways it limits you, and the adjustments that would let you work or study on equal footing. Sigmon Psychiatric writes accommodation letters for established patients across Arizona for a flat $179. New patients begin with a standard $179 evaluation, because a letter from a provider who has not evaluated you is one a disability office is right to question.

Who the letter is for

Conditions that commonly qualify include ADHD, anxiety disorders, depression, PTSD, bipolar disorder, and OCD.

What the letter contains

Many employers and schools have their own form instead of, or in addition to, a letter. The $179 fee covers either a letter or a completed form of ordinary length.

How the process works in Arizona

  1. Establish care. If you are new, book a $179 initial evaluation at sigmonpsych.com/book. Current patients skip this step.
  2. Request the letter. Text (520) 650-5111 or email sigmonpsych@gmail.com with the recipient (employer, school, testing board), any form they require, and the accommodations you are asking for.
  3. Receive it. Letters are typically ready within five business days and delivered securely as a PDF, or faxed to the recipient with your written authorization.

Accommodation decisions belong to the employer or school; the letter gives them the documentation they need to make a fair one. Arizona's public universities and community colleges each publish their documentation guidelines, and the letter is written to meet them.

What the letter is not

It is not a guarantee that any specific accommodation will be granted, not a disability determination for Social Security or long-term disability, and not a substitute for formal psychoeducational testing when a testing board requires it (as some do for learning disabilities). Angela will tell you when a request is outside what she can honestly document.

Assessments and letters are outpatient services, not emergency care. If you or someone with you is in immediate danger, call 911. For a mental health crisis, call or text 988 or Arizona's statewide crisis line at 1-844-534-4673, both available 24 hours a day.

Frequently asked questions

How much does an accommodation letter cost?

$179 for established patients, the same as a standard visit. New patients complete a standard $179 initial evaluation first, because an accommodation letter must rest on a documented diagnosis and treatment relationship. Most letters are completed within five business days of the request.

What conditions qualify for workplace or school accommodations?

Under the Americans with Disabilities Act and Section 504, a diagnosed mental health condition that substantially limits a major life activity can qualify, including ADHD, anxiety disorders, depression, PTSD, bipolar disorder, and OCD. The letter documents the diagnosis and functional limitations; the employer or school decides which accommodations are reasonable.

What accommodations are commonly requested?

Extended time on exams, a reduced-distraction testing environment, note-taking support, flexible start times, remote or hybrid work days, scheduled breaks, written instructions, and time off for medical appointments. The letter recommends accommodations that match your specific limitations rather than a generic list.

Can I get an accommodation letter at my first visit?

Not at the same visit. The initial evaluation establishes the diagnosis; the letter can be requested afterward and is usually ready within five business days. This sequence is what keeps the letter credible to a disability office.
