Accepted insurance plans

Coverage is verified on a per-patient basis. The following plans are commonly accepted:

• Blue Cross Blue Shield (BCBS)
• Cigna
• United Healthcare / UHC
• Humana
• Oscar Health
• Optum

Medicare and Medicaid (AHCCCS) are not accepted at this time.

Self-pay rates

• Initial psychiatric evaluation: $179
• Follow-up appointment: $129

HSA and FSA cards are accepted for self-pay visits.
