If you have one of these plans and would still like to be seen, you are welcome to book as a self-pay patient: $179 for the initial evaluation and $129 for follow-ups, with HSA/FSA cards accepted. Visits are not billed to Medicare, Medicaid, or AHCCCS, and the receipt cannot be submitted to them for reimbursement.
